They are talking about the gun holding a 4th shell in the receiver while still having the plug in the magazine that is supposed to hold only 3 shells. Benelli's and SX2 are guns that can do this and possibly others. Two shells are loaded in the magazine, one is in the chamber and one "floats" in teh receiver. You get four shots! Any gun with a magazine cutoff switch should be able to do that. After the second shot, disengage the mag cutoff and bang out the second pair.
