Ok, got a question. First time doing my taxes. I already sent out Federal, just have to send out state. Do I need to include one the W2 forms with my state tax paper when I mail it? State probably differs by state but I know you NYers should know. My state form is the Resident Income Tax Return (short form) IT-150. I have two W2 slips that say to be filed with employee's state, city, or local tax return. So do I include this? If so, do I staple it somewhere? Thanks!

Don't know about NY, but I'd bet you do. You might also check to see if you owe a local tax. A lot of school districts operate at least partially on an income tax, not a property tax.

Read the form or form booklet to see if you have to "attach" the W-2. Ohio, curiously, printed "please do not staple W-2" right on the form this year. I guess they got tired of pulling them out. grin.gif

No, you need to fill out a form which is labeled IT-2 in your booklet. This is the summary of federal form W-2 statement.
