He has signs of mange and a very sick deer that is for sure. I don't know whats with his special area. His left front leg is turned funny too. Feel bad for the poor buck though. I would say that he was hit by a car and it messed up his leg, lost some hair (don't think it is mange) and suffered a torn abdominal wall (or hernia if you will). His intestines are protruding through the muscle wall and are just being held in by his skin. He will probably suffer a slow death as he develops gangrene which looks like it may already be started.
