Things to note: 1. No personal information is collected by the survey, unless you enter it at the end. 2. 5 $25 gift cards will be given out for completing the entire survey. 3. The survey should take less than 5 minutes 4. It uses Google Forms, because its free, easy to use and has pretty graphs. If you hate Google, I am sorry but I am too poor to afford a real survey tool.
